1. Confirm Whether the Case Involves a Breach of the Personal Data (Privacy) Ordinance

2. Complete the “Privacy Complaint Form”

The details of what happened (when, who, where the disclosure occurred)

The type of information involved (for example, medical records, diagnosis, screenshots from WhatsApp)

3. Provide Supporting Documents (if available)

This may include doctor’s certificates, communication records, data collection forms, or audio recordings.
